Medical social work is an important part of CarePartners’ continuum of care. Their important role involves helping individuals, couples and families cope with social, psychological and medical issues resulting from illness.

Often, individuals do not know what resources are available or how to access them, and need the assistance of a qualified social worker. Whether there is a need for access to community resources, understanding post-hospitalization options, getting referrals to health care resources or discussing lifestyle changes, the social worker is there to offer guidance and answer questions.
