Do You Need Home Health Care?

There are many advantages to receiving care in the home. We invite patients, family members and health care professionals to contact us with inquiries about our services and to determine if an individual can benefit from becoming a CarePartners’ patient.

Patient Eligibility

To receive CarePartners’ home health services, patients:

• Must be under a physician’s plan of care
• Must be homebound (unable to leave home without great effort)
• Must require intermittent skilled service(s)
